Do not use Save/Install Bank to back up your question bank as you edit it. It is recommended that you use Download to make backup copies of your question bank while editing is in progress, and use Install Bank only after you are finished editing a question bank.
If you use Save/Install to save all incremental edits to your question banks, there are side effects:
The most recent saved copy is used in assignments, including tests. The questions in the edited question bank replace older versions in current assignments. This can create some confusion if some students have completed an assignment under an earlier version of the question. To resolve this situation, open the student's graded assignment record in the Gradebook and review results there. The Gradebook record always reflects exactly the questions and assignments that a student was presented at the time that the assignment was completed -- even if you subsequently edit the questions.
